S&B Application Forms for Credentials

Print off an application, fill it in, attach any requirement additional information, and mail with a check to Safety and Buildings Division, PO Box 78780, Milwaukee, WI 53293-0780. Sending an application via express or overnight delivery to the Madison East Washington Ave address does speed up processing. That adddress information is on the application forms. For further information, contact the S&B Credentialing Unit, 608-261-8500.

- Here is info on getting education courses approved for S&B credentials holders.

- SPS 305, License, Certification, and Registration Code, describes each credential.

- There are no credential renewal forms here because those forms are customized for each credential holder and sent out approximately 45 days before the credential expiration date. If you have not received an expected renewal notice by 30 days before the expiration date, send an email inquiry, including your current mailing address, to DspsSbCredentialing@wi.gov.

Thermal Insulator Credentials - Note: The budget bill signed July 26, 2011, eliminated the previous legislative mandate for three Thermal Insulator credentials. S&B hasl initiated the repeal of SPS 305.7, Licenses, Certifications, and Registrations Code, language for those credentials and will not be approving any new applications.
