Our LicensE Application Status Lookup tool makes it easier for employees, and helps academic advisors support students during the license application process. To access the Application Status Lookup tool, you will need the applicant's 10-digit PAR number. An applicant may provide the number to anyone who could support their application. Permit Information
No permit shall be issued by the Board unless the Funeral Establishment has in charge, a full time, licensed Funeral Director.
The facility must have preparation room with table, hot and cold running water and equipment, instruments and supplies necessary for embalming. See
Wis. Admin. Code § FD 1.09, for a detailed description of preparation rooms. Each Funeral Establishment must also have pricing cards, disclosure statement, itemized statement and casket listing. See the Federal Register Part 453 regarding funeral industry practices
