  • Anyone applying for an initial license for any of the credentials listed can apply at license.wi.gov.

Our LicensE Application Status Lookup tool makes it easier for employees, and helps academic advisors support students during the license application process. To access the Application Status Lookup tool, you will need the applicant's 10-digit PAR number. An applicant may provide the number to anyone who could support their application. Application for Licensure​

Application methods are listed below. Select the corresponding "Key Steps" link to view requirements. You may be asked to provide additional information during application submission or in your LicensE due diligence checklist once your application has been reviewed. All requirements originate from Wisconsin Statutes and Administrative Code.  ​​

​This application method is available to genetic counselors currently credentialed in another jurisdiction. The credentialing and renewal requirements for​the existing credential must be substantially equivalent to Wisconsin requirements.

​Application for Fee Reduction (Optional - If applying for a reduction of your credential application fee, this form must accompany the LicensE application for the credential. Renewal applications are not eligible for a fee reduction.)
Documentation of Name
​Name Documentation (if applicable) There is no form for this requirement. However, if your name on all credentials or incoming supporting documents is not the same submit a certified copy of marriage certificate, divorce decree, etc.​

​Application for Predetermination (Optional - Submit ONLY if you have been convicted of any felony, misdemeanor, or other violations of federal or state law in this state or any other and you desire a review of your conviction record before submitting a credential application.)
