Overview
This page is the subsite for schools that are reapplying after any licensure lapse due to a license revocation, voluntary license surrender, board order specifically requiring reapplication, or five years have passed since previously-held license expired. Application requirements for this method, including applicable forms, can be found on this page.
Before You Apply
You should submit your application at least 45 business days prior to the anticipated opening date.
To obtain an Electrology School credential, you must first register as an Organization on LicensE. You cannot apply for licensure as an Electrology School through the same account that you use to manage your personal licensure.
How to Apply
Below are some general instructions for applying for a credential on LicensE.
Create or log into your LicensE account. Please visit LicensE account Information page for detailed guidance on selecting your account type.
Select "Apply for a License."
Select your Category.
Select your License Type.
Select your Application Method, then "Next."
Review your Personal Information.
Enter the requested Information on each page of the application. Information may include Examination, Education, or License History, depending on your profession.
If applicable, request a Veterans Fee Waiver or Initial Credential Fee Reduction.
Submit the requested Documents you may already have. You do not need to have all documents ready to submit.

Respond to the Attestation questions.
Provide Payment Information. Fees may be found Credential Fees.
Submit your application.

Application Information
After logging in to your Organization account, select Apply for a License and choose:
Category: Business
License Type: Manicuring School
Application Method: Renewal After 5 Years
Please Note: You are not required to provide all documents before initial application submission. You do not need to upload documents to every available category.
You must provide the names and address of all owners. If the owner is a corporation, a copy of the articles of incorporation and most recent annual report filed with the secretary of state, together with a list of the names and addresses of the incorporators, officers, directors, and shareholders of the corporation shall be submitted. No identification is required of any shareholder owning or controlling fewer than 10% of the shares of the corporation. If the owner is a partnership, the names and address of all partners and a copy of the partnership agreement must be submitted.
Submit a list of the names and applicable license or certificate numbers of the instructors hired to provide practical and theory instruction to their students.
Submit the name and Wisconsin address of a designated agent upon whom any process, notice, demand, or other document may be served.
Submit a statement that a certified public accountant has completed financial statements, consisting of a balance sheet, a profit and loss statement, and a cash flow statement, and that a copy of those statements will be made available for inspection by the Department on request at any time. The Department may require an audit of the finances of a school, at the applicant's expense, if the licensing application contains information, which reasonably gives rise to concern that the school is not financially sound or is not otherwise stable.
Submit a detailed floor plan of the premises to be used by the school, which identifies the site of the school and includes a description of fixtures and equipment.
Submit a list of the names and addresses of all schools of any description whatsoever currently or previously owned or operated by any of the owners of the school applying for licensure. If the owner of the school applying for licensure is a corporation or partnership, a list of the names and addresses of all schools of any description whatsoever which are currently or were previously owned or operated by any of the incorporators, officers, directors, or shareholders of the corporation or partner. No report is required for any shareholder owning or controlling fewer than 10% of the shares of the corporation.
For every school listed, a detailed statement with all pertinent details indicating:
Whether the school was ever denied accreditation by any accrediting agency;
Whether the school was ever denied a license or had a license suspended, limited, restricted, or revoked;
Whether the school was ever subject to any form of receivership; and,
Whether the school was ever the subject of or named in any lawsuit alleging fraud, misrepresentation, or violation of law.
Submit a copy of the printer's proof of the catalog or bulletin as described in Wisconsin Administrative Code. (See link below)
Submit a copy of any contract form used by the school or which the school intends to use in enrolling students per Wisconsin Administrative Code. (See link below).
Submit the number of students anticipated to be enrolled during the license period and a Surety Bond.

Schools of barbering, cosmetology, aesthetics, electrology or Aesthetics needing designation as a postsecondary institution for purposes of federal Title IV funding need to complete the Addendum to Application (Form #5555).
The form must include proof of an admissions policy that requires applicants to possess a high school diploma, GED, HSED or apply through a dual-enrollment/dual-credit program in compliance with Wisconsin Statute.
Upon receipt and review of the form and accompanying documentation, a license will be issued indicating the designation.

What Happens Next?
After you upload all required documents, submit your application, and pay the required fee(s):
Preliminary Application Reference Number (PAR)
Your application will be assigned a Preliminary Application Reference number or PAR number. This number allows you and trusted parties to look up the status of your application without logging in. You can also reference this number in any support tickets you submit in regard to your application.
A trusted 3rd party can look up your credential status by visiting LicensE, clicking “Application Status Lookup” and entering the PAR number you have provided to them.
Review Timeline
Your materials will be reviewed within approximately 10 days by a member of the credentialing team.
Note: Some licenses do take longer to process due to complexity. You can see the performance data on our LicensE Dashboard to see what date we are reviewing for certain application types under "Application Review Times by Profession."
Pending Applicant Input Status
After this review, if anything is missing or the credentialing team has any questions on your application, your application will go back into a status called “Pending Applicant Input.” You will receive an email letting you know your application was moved into this status. You should then log into LicensE and check your application “Due Diligence.”
To check your "Due Diligence"
Your Due Diligence is a checklist of items required for your credential.
The Due Diligence section is how DSPS staff will communicate with you regarding your credential requirements.
Log in to your LicensE account
Click on "Check Application Status"
Your credential application should be shown on this page. Under “Action” select the button that looks like an upside-down triangle. If you are on mobile, you will need to turn your phone into landscape view. Note: LicensE is best viewed on a desktop or laptop.
After clicking the upside-down triangle, select “View Due Diligences.”
The Due Diligence column lets you know what the requirement is.
The “STATUS” column will you know if the item is “Met”, “Not Met”, or “Not Applicable.”
Items listed as “Not Met” are items that are needed by the Department.
The “NOTES” column of your Due Diligence will provide further written details or instructions about what is still needed.

Log into your LicensE account
Click on "Check Application Status"
Your credential application should be shown on this page. Under "Action" select the button that looks like an upside-down triangle. If you are on mobile, you will need to turn your phone into landscape view. Note: LicensE is best viewed on a desktop or laptop.
After clicking the upside-down triangle, select "Upload Document."
The document must be an allowable document type.
After uploading the document, click "submit." If you do not, it will not be sent to a reviewer.
If you have any questions, you can submit a support ticket on LicensE by logging in and clicking "request support" at the top of the page.
LicensE statuses that require action from applicant:
Status | Description |
Pending Applicant Input | The application is with the applicant to submit additional documentation and/or information. |
Pending Exam | The Department is waiting for the applicant to complete a required examination and receive passing results. |
LicensE statuses that indicate the application is with Department staff:
Status | Description |
Credentialing Team Review | The application is in the queue of a DSPS team member and awaiting review. |
Legal File Preparation | The application is with a member of the legal team who is compiling arrest/conviction documents for review by an attorney. |
Legal Review | The application is with an attorney, or another member of the legal team who performs the actual review. |
Board Review | The application is with board liaison and/or the board for determination. |
Awaiting Regulatory Authority | The application sent to regulatory authority (Chief Legal Counsel) for determination. |
Discipline and/or Monitoring | Application is with legal staff but will be held until Division of Legal Services and Compliance disciplinary matter is resolved. |
