Health Profession Accommodations
National Examination
Special accommodation requests for the National Examination for Speech-Language Pathology and Audiology (PRAXIS) are requested and submitted directly to the American Speech-Language-Hearing Association (ASHA).
State Examination
Create your application in LicensE. Please check the box requesting exam special accommodations when completing your application and pay the application fee.
Complete and upload to your application in LicensE the following forms:
Form 2349 Requests for Modification of Examinations for Persons with Disabilities
Form 1907 Disability Modification Request Form for Examinations
Form 2350 Professional Verification of Request for Modification
Note: Form 2350 should be completed by your medical professional, and they should upload to your application in LicensE. Make sure to note your application number on the form.
Upload your accommodation documentation as early as possible. Applications must be received, processed and approved 30 days prior to the next exam date.

Licensed Professional Counselor (LPC)
Applicants are required to successfully complete either the NCE, NCMHCE, or CRCE.
Please note: Our department does not authorize applicants to take the NCE. If you have registration questions, please contact NBCC online or at (336) 547-0607.
Note: Candidates requesting special accommodations must obtain DSPS approval prior to scheduling their exam. Once approved, DSPS will send the approval to the NBCC.

NCLEX testing accommodations for otherwise qualified candidates can be provided only with the authorization of Wisconsin Board of Nursing.

Register for your exam with Pearson, pay the fee, but do not schedule an appointment to take the NCLEX until you have received written confirmation of your accommodations and your ATT. Accommodations cannot be added to an already schedule exam appointment.
You will receive an email listing the granted accommodations.
Once you have received the approved accommodations email you may schedule your exam appointment online with Pearson through the NCSBN website.
Psychologist
Examination EPPP testing accommodations can be provided only with the authorization of Wisconsin Psychology Examining Board.
Create your application in LicensE. Please check the box requesting exam special accommodations when completing your application and pay the application fee.

Register for your exam with the ASPPB, pay the fee, but do not schedule an appointment to take the EPPP until you have received written confirmation of your accommodations and your ATT. Accommodations cannot be added to an already schedule exam appointment.
You will receive an email listing the granted accommodations.

Pearson Special Accommodations
Exam accommodations requests must be directed to Pearson . Please submit your request via the website: Request test accommodations
It can take up to ten days for an accommodation request to be approved. After the request is approved, the candidate can proceed with scheduling.
Accommodations cannot be added to already scheduled exams. Candidates must wait to schedule.

Trade Exam Special Accommodations
Accommodations for DSPS Exam Locations
To request special accommodations for a trades exam at a DSPS location, please complete the following forms and upload them with your LicensE application. Please check the box on the LicensE application indicating your request for exam accommodations.
For pending applications, please send the forms to DSPSCourseApproval@wisconsin.gov.
Please note that both forms must be submitted for consideration.
