A National ID is required for MMA competition. If you are a debut fighter or your ID has expired, you can apply for this ID

There is no form. Upload results obtained from the lab or physician into your LicensE application.
No, that is considered an interpretation. The actual blood work labs paperwork the doctor referenced is also required.
Wisconsin requires that physicals and blood work be 180 days old or less at the time of licensing.

No, licenses are good for one (1) year from the date it is issued. You can use your license to compete in a Wisconsin
different event until it expires.
A Second/Corner license is required for anyone who expects to enter the locker room, wrap hands, and/or be at cage or
ringside to coach.
No. For the safety of all, family members, including children, must remain outside of the locker rooms. Corners/Seconds must be 16 years or older to be licensed.
