Carry your credential and your completed SBD-10889, Evidence of Completion of Welding Tests, card on your person. Make cards immediately available upon request and mark welds as required.

The following additional documents may be requested at any time:

• Welding Procedure Specification (WPS)

• Procedure Qualification Record (PQR)

• Welder Performance Qualification (WPQ)

• Welder Continuity Record

How to Obtain Welder Registration

1. Obtain a Welder Registration Application

2. Consult with a Wisconsin Weld Test Conductor. Certified Weld Test Conductors are listed on the Welder page

3. Perform weldment of test coupon

4. After satisfactory test, obtain results with the conductor's signature

5. Submit credential application to DSPS via license.wi.gov.

Inspections

Any authorized representative holding a valid DSPS credential and given the responsibility to enforce structural welding requirements of the commercial building code may perform structural inspections.

Inspectors may be employed by DSPS, the county, municipality, or state-contracted authority. The individual could be a local builder, boiler, elevator, or other public safety official.
