Before You Apply

To gain privilege to practice, you must apply on both the CompactConnect system (link below) and on LicensE.

Your application via the CompactConnect system is considered notification of your intent to practice in Wisconsin as required by state statute.

How to Apply

Below are some general instructions for applying for a credential on LicensE.

Application Information

After logging in, select Apply for a License and choose:

Category: Health
License Type: Occupational Therapy Assistant
Application Method: Privilege to Practice
Notification to the Compact: If you have already applied via CompactConnect, select “yes.”

Please note: You are not required to provide all documents before application submission. You do not need to upload documents to every available category.

Occupational therapy assistants practicing in Wisconsin via a Privilege to Practice must supervised by an occupational therapist licensed or holding a compact privilege.

Complete and submit a Form 8166 Occupational Therapy Assistant Supervision Form (link below)

An FBI Background Check is required for participation in the Compact. This check is typically completed by your home state as part of the Home State Approval process.

If for any reason your home state did not conduct a background check, you are required to notify Wisconsin during this application process. A background check will then be conducted by Wisconsin in order to grant your privilege to practice.

If you hold an active compact privilege to practice and wish to convert your privilege to a Home State Approval, please see the Privilege to Practice to Home State Conversion application method.

Exam Information

You must pass the open-book examination on the Wisconsin Statutes and Administrative Code in order to gain privilege to practice.
