Physician - Interstate Medical Licensure Compact

To be eligible for participation in the IMLC, you must meet the IMLC’s eligibility requirements. Additional information regarding IMLC eligibility is available on the IMLC website (link available below).

All physician licensure and renewal through the IMLC will begin at the IMLC website (link available below).

All applications for IMLC licensing are completed through the IMLC’s online portal on their website (see link below). Once an application is submitted, it will be sent to our department for review. If additional information is required, DSPS staff will reach out with further details. Information regarding the different application types is available below, and further information is available on IMLC’s website.

If you have a current, active Wisconsin physician license and are interested in participating in the IMLC, review the IMLC’s eligibility requirements to determine if you qualify. Your Wisconsin physician license must be current and active before you request a a Letter of Qualification (LOQ). You will then complete an application for an LOQ directly through the IMLC.

When we receive your application from the IMLC, we will email you instructions regarding next steps. All applicants are required to complete the following:

• Fingerprints – Instructions will be emailed upon receipt and review of LOQ application from the IMLC.

• Medical Examining Board Compact Affidavit (Form 3221) – Complete form and email to our department at IMLCompact@wisconsin.gov 

• Authorization for Release of FBI Information (Form 2687) and recent photograph - Read the instructions and complete the form. The Race/Ethnicity/Sex information is required for your criminal background check. Form 2687 and your photograph may be emailed to our department at IMLCompact@wisconsin.gov.

Additionally, you may also be asked to provide the following:

• Transcripts of your national exam scores (USMLE, COMLEX, NBME, FLEX)

• Verification of your primary residence

• Hospital, Facility, and Employer Verification (IMLC) (Form 3202) - Complete the applicant information and forward to all hospitals, facilities, and employers in the state of Wisconsin

• Verification of federal tax filing

Questions and clarifications about your application should be submitted as support tickets in LicensE. Email addresses provided for document submission are not monitored for support purposes. After a document is emailed, you can submit a support ticket to alert our staff.

If you hold an active LOQ in another state, you may apply to add a Wisconsin physician license. This process is initiated directly through the IMLC. When your license is issued, it will appear in both your IMLC portal and in our LicensE system.

To expedite the process, you may consider registering for a LicensE account by going to license.wi.gov and selecting “Register as an individual.” If you do not register for a LicensE account, DSPS staff will create one for you when your license is issued.

If you hold an active, unrestricted medical license in Wisconsin, you may be able to redesignate your State of Principal Licensure (SPL) to Wisconsin. To do so, submit a redesignation application directly through the IMLC. To be eligible for redesignation, we will verify one of the following criteria:

• Primary residence in Wisconsin
• At least 25% of practice of medicine in Wisconsin
• Employer located in Wisconsin
• Wisconsin as the state of residence for US Federal Income Tax purposes

If you hold a physician license obtained through the IMLC, you must renew online through the IMLC. Only licenses obtained through the IMLC can renew through the IMLC site.

If your physician license obtained through the IMLC has expired and you are still eligible to participate, submit a support ticket in LicensE requesting our staff to have the IMLC make your previous license available for renewal.

If you are no longer eligible to participate or renew via the IMLC, you will need to apply for a traditional Wisconsin physician license. Information regarding this application process is available under the Initial Licensure subpage.

Your materials will be reviewed within approximately 10 days by a member of the credentialing team. Application status questions should be submitted by support ticket in LicensE.

LOQ Applicants - Applicants will be emailed instructions regarding required next steps for their application, including fingerprinting instructions and necessary forms.

Applicants for Wisconsin Licensure - You will be notified by email when your license is issued. If further information is required, staff will contact you by email.

Redesignation Applicants - You will be notified by email when your redesignation is completed. If further information is required, staff will contact you by email.

Renewal Applicants - You will be notified by email when your license is renewed. If further information is required, staff will contact you by email.
