Category: Unarmed Combat Sports.

License Type: (select from the applicable list below).

• Boxing Judge.

• Mixed Martial Arts Judge.

• Muay Thai Judge.

• Kickboxing Judge.

Application Method: New Licensure.

Supporting Documents you will be asked for:

1. One or more of the following:

   1. A certificate of completion of a judge's training program from another state, other regulating bodies such as the Association of Boxing Commissions, and other organizations that have a judge's training program certified by the Association of Boxing Commissions or another association recognized by the Department. 

   2. A resume with three (3) professional references that can verify the number of years of experience as a judge along with a log of experience. 

   3. A valid and current license as a judge from another state or organization. 

   4. A passing grade on an examination administered by the department that tests the examinee's knowledge, and successful completion of the trial judge program as denoted in the Wisconsin Administrative Code. 

   5. The trial referee program administered and supervised by the Commissioner, inspector, or Department representative shall consist of all of the following:

      1. Observing unarmed combat sports events. 

      2. Shadowing a licensed judge at unarmed combat sports events.

      3. Officiating, on a trial basis, as a judge during an unarmed combat sports event under the supervision of the commissioner, inspector, or Department representative.

2. Judge Eye Examination Report (required)

   1. Upload completed Judge Eye Exam report to your application.

3. Requests for additional information may appear in your LicensE Due Diligence list after your application has been submitted and reviewed.

Category: Unarmed Combat Sports.

Application Method: New Licensure.

   1.    5. A trial referee program administered and supervised by the Commissioner, inspector, or Department representative shall consist of all the following

      1. Observing unarmed combat sport events. 

      2. Shadowing a licensed referee at unarmed combat sports events. 

      3. Officiating on a trial basis, as a referee, during an unarmed combat sports event under the supervision of the Commissioner, inspector, or department representative. 

2. Referee Medical Examination Report (required). Upload the completed form (see the forms section below) into your LicensE application. 

Category: Unarmed Combat Sports.

License Type: Timekeeper.

Application Method: New Licensure.

The Department may determine whether a person possess the knowledge and experience necessary to hold a license as a timekeeper by successful completion of the trial shadowing timekeeper program administered and supervised by the commission, inspector or Department representative. The trial timekeeper program may consist of one of the following:

1. Observing unarmed combat sports events.

2. Shadowing a licensed timekeeper at unarmed combat sports events.

3. Timekeeping, on a trial basis, during an unarmed combat sports event under the supervision of the commissioner, inspector, or Department representative. 

Supporting Documents you will be asked for:

If you are a licensed timekeeper in another state (upload all of the following into your LicensE application).

1.  Copy of current license in another state.

2. Resume.

3. Enrollment in or completion of a timekeeper program. 

The Department may request additional information necessary to determine an applicant's eligibility for a license.

Category: Unarmed Combat Sports.

License Type: (select from the applicable list below).

• Ringside Physician

Application Method: New Licensure.

Supporting Documents you will be asked for:

1.  Application and Fee(s): Apply and pay fee(s) online in LicensE.

   • If needed, you can save your application and submit it later. Fees are $10.00 per year. (You may apply for up to five (5) years.

2. You must hold a license to practice in Wisconsin for not less than 5 years. Physician means a person licensed to practice medicine and surgery under Wis. Stat. § 448.04(1)(a) (Wis. Stat. § 444.01(1m)).

3. Your application must include your Social Security Number (SSN). If you do not have an SSN, Form 1051 is required.

Below are some general instructions for applying for an Unarmed Combat credential on LicensE. 

1. Create and account for new users  or log into your existing LicensE account using your email as USERNAME.

2. Select "Apply for a License."

3. Select Unarmed Combat Sports.

4. Select your License Type.

5. Select your Application Method (Always choose NEW license, never renew), then "Next."

6. Review your Personal Information.

7. Enter the requested Information on each page of the application.

8. Submit any requested Documents you may already have. You do not need to have all documents ready to submit.

9. Respond to the Attestation questions.

10. Provide Payment Information. Fees may be found Credential Fees.

11. Submit your application.
