Supporting Documents you will be asked for:

1. You must enter your Social Security Number (SSN). If you do not have an SSN, complete form 1051 linked below. 

2. Include in your application

   1. The name of the contestant (fighter) you will support.

   2. Name of the gym.

   3. Upcoming contest (event) date, if available. 

Important Information:

1. A maximum of 3 licensed seconds will be allowed to assist any one contestant or be positioned in a designated area by ring or cage during a non-championship bout. 

2. For championship bouts, there may be four (4) licensed seconds allowed to assist any one contestant. 

3. The appropriate number of licensed seconds allowed for championship and non-championship bouts will be subject to the approval of the Commissioner or Department representative and based on venue size and space. 

Below are some general instructions for applying for an Unarmed Combat credential on LicensE. 

1. Create and account for new users  or log into your existing LicensE account using your email as USERNAME.

2. Select "Apply for a License."

3. Select Unarmed Combat Sports.

4. Select your License Type.

5. Select your Application Method (Always choose NEW license, never renew), then "Next."

6. Review your Personal Information.

7. Enter the requested Information on each page of the application.

8. Submit any requested Documents you may already have. You do not need to have all documents ready to submit.

Note: Forms are now hosted on AccessGov. Visit AccessGov Forms Directory to choose your form.

9. Respond to the Attestation questions.

10. Provide Payment Information for the $40/year fee You may apply for up to 5 years. Please DO NOT use ACH if your event is sooner than 2 weeks.

11. Submit your application.
