Tattoo and Body Piercing Establishments - General Licensure

• You are registering for a tattoo, body piercing, or a combined tattoo and body piercing establishment license for the first time, or your tattoo and/or body piercing establishment has undergone a change in ownership or location for an existing license.

• You have confirmed that your establishment is not located in a county or city that is a delegated agent. Refer to Body Art program page linked below.

1. Review the body art program page linked below to determine if the city or county your establishment is located in is a delegated body art agent.

2. If your county is listed as a Wisconsin Municipality Delegated as Body Art Agents:

   1. DO NOT submit an application to the Department of Safety and Professional Services.

   2. Contact the agent in that county for the application instructions.

3. If your county is NOT listed as a Wisconsin Municipality Delegated as Body Art Agents on the body art program page:

   1. Go to our establishment licensing system, My Health Department, linked directly below. 

   2. Create an account or log into an existing account

   3. Submit an application​

      1. To submit payment, complete form 3071 and upload it as a file during the application process. Note: Please have the form ready upon application. You will not be able to add it later. 

4. Schedule Pre-Licensing Inspection: Once your application has been received and processed, a DSPS Inspector will contact you to schedule a Pre-Licensing inspection. During the inspection, the Inspector will share with you the applicable code requirements that your establishment must meet.

Your establishment application will be reviewed, and you will be emailed at the email address provided on the application if more information is needed. 

Once all required information is received. Your application will be assigned to our inspection team who will contact you directly for inspection scheduling.
